How to Apply for an e-UDID Card?

Applying for an e-UDID card is a simple online process through the official UDID portal. You just need to fill in your personal information and submit your application for verification. Follow these steps

  • Step 3: Choose whether you want to apply using your Enrollment Number or UDID Card, then click on ‘”Submit’.

Step-by-Step Guide to the UDID Card Renewal Process:

  • Step 1: Visit the official UDID website: https://www.swavlambancard.gov.in/
  • Step 2: Click on ‘Apply for Disability Certificate and UDID. Add your personal information, like employment, identity, and disability details. Also, upload your necessary documents, like Aadhar and address proof, and importantly, your disability-related document. Upload your documents as mentioned on the website and click on submit.
  • Step 3: Your application is sent to the chief medical officer (CMO) or the district medical officer (DMO), and the CMO Office/Medical Authority verifies your data.
  • Step 4: The CMO or DMO Office/Medical Authority assigns the concerned specialist doctor for an opinion.
  • Step 5: The specialist doctor assesses the disability of the PwD and will submit the assessment report to the medical board.
  • Step 6: Number Six, the Medical Board will decide on the type and percentage of disability, the validity of the disability certificate and other issues that the applicant might have.
  • Step 7: The CMO or the DMO will inform the board of their decision. The CMO Office prepares the Disability Certificate and generates the UDID and Disability Certificate.
  • Step 8: Generate the Card and Disability Certificate based on the assessment details. The UDID datasheet goes for UDID card printing, and the card is dispatched to the PwD.

What are the different types of UDID cards?

The UDID cards are basically divided into 3 types depending on disability level.

White

Your UDID card is white if your disability is less than 40%.

Blue

Your UDID card is blue  if your disability is More than 80%

Yellow 

Your UDID card is white if your disability is 40%–80%

What is the validity of a UDID card?

Validity depends upon whether it is a permanent or a temporary card. 

Permanent

When there is no progress or regression in your disability. It’s a Permanent.

  • Please note that in case of correction in name, date of birth, photo, address, etc., an appeal mechanism is not required. The PwD can use the mechanism after logging into the Swavlamban portal, and reassessment is not required.
  • This SOP is only for original disability certificate & UDID card-related grievances. A separate SoP is being issued for appeals against the decision of the medical board in case PwD applies for higher education and government jobs, etc., through the PwD reservation quota.
  • All offline practices of appeal in a State/UT shall stop once that State/UT moves to the online appeal process.
